Home and Community Care- Program for Younger People- HACC PYP - Macedon Ranges Health

Macedon Ranges Health delivers the Home and Community Care Program for Younger People (HACC-PYP) in the Macedon Ranges Shire. This program is funded by the Victorian Government and is designed to support eligible individuals from the ages of 0 to 65 years old, to stay independent at home and in the community by providing them access to a range of affordable health care and support services.

Services may include time limited access to meals assistance, home maintenance, domestic assistance, social support, and allied health services (e.g. physiotherapy, occupational therapy).

What does this mean for me?

It might be some extra supports:

  • During/after an illness or injury
  • During a period of medical treatment
  • If you are experiencing financial hardship which is impacting your      health
  • If you are experiencing mental health concerns
  • To support you whilst you are focusing on a carers role

If eligible for HACC-PYP the Victorian Government will cover the cost of a range of services, ensuring you can access the support you need. You may be required to make a small contribution to the cost of your services. This fee will vary depending on your income and current circumstances. At assessment we will chat with you to determine your income and factors that may affect your ability to pay fees. Fees may be waived in circumstances of financial hardship.

Referrals can be made by yourself, a family member (with your consent), your GP, or any other health professional.
